About Us

Health Plus Home Health Care is committed to providing safe, high quality care and to continuous performance improvement in all that we do. We recognize that it is the combined efforts of our staff, patients, caregivers, family and referring physicians that optimal patient care and recovery occurs. Liz Tucker is our Director of Patient Care. She has extensive experience in providing excellence in home health care to the community of San Diego. Through years of service, she has developed an understanding of true patient-centered care. She believes that optimal home health delivery occurs when the patient, family and staff collaborate to fully understand the individual patient and their needs. Liz works closely with our referral sources and staff to facilitate and promote our integrative team approach to home health.

Susan Trebon is the Administrator of Health Plus Home Health Care. Her 25 years of providing direct patient care, with 15 of those years in home health, gives her the experience and knowledge to give exceptional leadership to our staff. Our patients deserve the best and she insures that our trusted staff has the professional skills, knowledge and training to provide care that exceeds the expectations of our patients, their families and referring physicians.

Patricia Cooper, CPAPatricia Cooper, Business Operations
Certified Public Accountant (CPA)

Patricia Cooper is responsible for the overall business administration of Health Plus Home Health Care. With 23 years as a Certified Public Accountant, her training and experience ensure that agency operations run smoothly. Whether she is assisting clients with insurance claims or preparing our annual budgets, her dedication and expertise is evident in her daily work. Patricia provides superior organizational leadership to our agency.
